If you have one of the following conditions which require monitoring, you will be invited to attend the surgery once a year for a health review with either the Nurse or the Doctor.

• Asthma
• Chronic obstructive pulmonary disease (COPD)
• Dementia
• Diabetes
• Heart disease
• Hypertension
• Kidney disease
• Mental health
• Peripheral arterial disease
• Rheumatoid Arthritis
• Stroke/TIA

If you do not want to be invited to attend a clinic to monitor your health, or, have previously declined to attend or not attended the appointments, but would now like to be invited to the clinics please complete the relevant form to let us know.

• Decline Annual Monitoring
• Attend Annual Monitoring
