If you would like to register your interest in the seasonal flu vaccination please complete this form, we also ask that you use this form to decline the vaccination should you feel you do not wish to attend this year. The Hollies Medical Practice can only vaccinate children with existing medical conditions.

Flu Vaccinations FAQs 2020
Our Flu Clinics this year will run from Mid-September for eligible patients.
Due to COVID-19 and the extra time and space the surgery will need to allow for these vaccinations, we will be running multiple clinics in which we will only be carrying out vaccinations.
