Does the lesion have an irregular shape?
For example, uneven, asymmetrical, or jagged edges
Does the lesion have more than one colour?
(For example, different shades of brown, black, red, white, or blue)
Approximately how large is the lesion at its widest point?
Has the skin around the lesion become inflamed?
For example, red, swollen, or sore
Has the lesion ever oozed, bled, or formed a scab or crust?
Have you noticed any change in sensation in the lesion?
