If you do not have a Patient Access account, you can use this form to request any prescriptions from the practice.

For Repeat Medications please allow 2 working days before contacting your nominated pharmacy to collect your prescription.

For Acute Medications please be aware they may not be issued without you speaking to your GP. It is important you read any responses sent to you with regards to your medication request.

In future you may wish to consider registering for our Online Services. The Online Services system remembers which medications you are on and makes requesting repeat prescriptions faster and easier.
