What is social prescribing?

Social prescribing is a service provided to GPs that links patients to community groups and social support.

Social prescribing works for a wide range of people, including people:

  • with one or more long-term conditions
  • who need support with their mental health
  • who are lonely or isolated
  • who have complex social needs which affect their wellbeing

Taking a personalised approach, the Social Prescriber works with the individual and family to develop a holistic support plan, which is then worked through in partnership to enable the person to achieve their desired outcomes, goals and aspirations.

Please note that your request will be assessed by a GP or other healthcare professional. This may result in a receptionist calling you to arrange an appointment, or the GP may call you to attempt to deal with your medical issue. If necessary a face to face appointment can be arranged after this call.
