Application for access to medical records

Use this service to request access to your medical records

Patients have the right, under the General Data Protection Regulation and Data Protection Act, to request access to, or gain a copy of information an organisation holds about them. This information may be held on a computer, in paper form or by any other new or existing medium or media. This is called a Subject Access Request (SAR). This form must be completed before any requests will be considered.

Patients should read the subject access request information page prior to completing this form.

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
