Requests for patient data from a third party

We kindly ask that all third party NHS services use this form to request patient data.

We will query all third party requests that do not come from a @nhs.net or @nhs.uk email address as part of our GDPR guidance.

We require consent to share data with any non-NHS party.

Please be advised that the patient’s date of birth will need to be entered on any replies from the practice to view the response.
