Important Information About Fit Notes
Please be aware that submitting this form does not guarantee that a GP will issue a fit note for you. A fit note is a legal document, and it’s important for us to ensure it accurately reflects your medical condition and needs.
You may be asked to book an appointment before the note is issued so that we can properly assess your condition. Our priority is to ensure all notes are issued accurately and meet legal requirements.
Please note that GPs can only backdate fit notes if they have factual information to support their decision. They cannot certify something they cannot verify.
If you’ve been off sick for more than 7 days
To request a sick note:
- fill out a sick note request form
If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
