District Nurses and CIT
The District Nursing team are no longer based with the Hollies Medical Practice at The Tamworth Health Centre. District Nursing is part of Community Nursing and provides nursing and end of life care to patients in the community. District nurses visit patients in their own homes/residential care homes, assess their healthcare needs, monitor the quality of care they receive, support their family members and are professionally accountable for delivery of care.

Health Visiting and School Nursing teams are here to support parents, carers and young people to be healthy. We provide advice and support right through from
pregnancy up until a child is 19. All families will be offered a number of routine contacts, plus extra visits where needed.
