Using an online form
There are many things you can do online at our surgery. Some tasks you can do using our website where you can:
- request an appointment
- request a sick note
- ask about test results
- contact us about an admin request
You do not need an account to use this website.
Using your NHS account or Patient Access
There are other tasks you can do online, like:
- order repeat prescriptions
- view your test results
- access your health record
- check or cancel appointments
- see your vaccination record
For these, you can use your NHS account (through the NHS website or NHS App) or Patient Access.
Setting up an NHS account
To set up your NHS account for the first time, you will need to set up an NHS login. Read about how to set up NHS login.
Once you have set up your NHS login, you can log in to your NHS account or download the NHS App onto your phone.
Using the NHS app and your NHS account
Your NHS account gives you access to certain NHS services online. You may be able to download the NHS App if you have access to a smartphone or tablet. You can log into your NHS account using the NHS App or the NHS website.
The app is designed and operated by the NHS in England to give you access to a range of services including repeat prescriptions, immunisation history and test results.
Setting up Patient Access
To set up Patient Access for the first time:
- fill out a register for online services form
- upload or visit the surgery with photo ID so we can confirm your identity
- we will give you a username and password
- go to Patient Access or use the app to register
Using Patient Access
How do I register for Patient Access?
You must be a fully registered patient to register for online access. You can request access by completing the register for online services form.
You will need to provide some photo identity i.e. passport/driving licence (if you haven’t already done this previously when you registered as a patient). Once we have verified your identity we will email you a separate access code and registration instructions.
Once you have the access codes and instructions you need to connect to the website and register for the service before the date on your instruction sheet expires. Once this date has passed you will need to collect new access codes in order to register for this service.
How do I get on to Patient Access?
You can visit www.patientaccess.com. The first time you use the site you will be asked to create your account. Here you will need to enter the four registration codes given to you by the practice.
Repeat prescription requests
If one of our doctors has arranged a repeat prescription for you, and the review date has not expired, then you can use Patient Access to request a repeat.
Prescriptions can be collected two full working days after request. If you have logged onto Patient Access you can view any medication items by selecting list of your repeat medication in the repeat prescriptions section of the screen.
Tick the items you require and then select make request. There is no need to add a message relating to your request. You will then see a confirmation screen, providing you are happy with the selection you made, select confirm.
Making appointments
Please ensure that you book the appointments appropriately. If you are unsure as to whether it is appropriate for you to see a nurse or doctor please contact us by telephone. When you book an appointment on-line you will receive confirmation. There is no need to re-confirm by email or telephone – just arrive promptly on the day.
Due to the nature of nurses’ appointments we are unable to offer them on-line.
We would ask that you enter a reason for your appointment in the box provided when booking your appointment as this gives us the opportunity to ensure that it is appropriate for you to see the doctor rather than a nurse. Please be assured that all details entered are secure and cannot be intercepted. Our practice has a strict confidentiality policy.
You can also use this facility to cancel appointments which will help us to offer them to someone else who may need one.
